Participation

For the INSPECT study we are looking for PHTS patients of all ages who want to participate in the study.

Participating in the INSPECT study means that:

◾ you will receive a digital survey to complete on the computer;

◾ we ask for your permission to request your medical data, or the medical data of the person you care for , and to use it encrypted

◾ you do not have to visit the hospital for this study.

After registration, we can send you more information about the INSPECT study and the consent form.

If you decide to participate in the INSPECT study, we will ask you to sign the consent form and return it to us. We will then send you the digital survey.

The information you enter above will be sent by e-mail to a secure email box of the Radboudumc. The data will not be stored on this website.

We save your personal information to be able to perform the INSPECT study, in accordance with the applicable legal requirements.

The purpose of processing your data is:

◾ to register your consent to participate in the INSPECT study;

◾ to send you a questionnaire;

◾ and to conduct the INSPECT study as described in the patient information.

If we want to process your personal data for a different purpose, we will ask you for your permission again. We store your data no longer than necessary to perform the INSPECT study, unless we must store your data longer according to the law. We do not share your data with third parties.
